Collection, Recording, Storage and Retention of Personal Information to Universiti Sains Malaysia Students;
Universiti Sains Malaysia is committed to comply with the Personal Data Protection Act 2010 ("the Act"). To comply with the Act, Universiti Sains Malaysia is required to manage the personal data that we collect from the students.
The purposes for which your personal data may be used are, but not limited to:-
| a) | administer and conduct assessment of your application to get the sponsorship/scholarship/PTPTN; |
| b) | Universiti Sains Malaysia's Resume Bank; |
| c) | administration purposes |
| d) | conduct the student's activities |
| e) | security and fraud prevention purposes; |
| f) | internal record keeping; |
| g) | statistical analysis; |
| h) | comply with the AUKU 1971 and USM students rules and regulation |
| i) | For the purposes of our corporate governance (collectively, "the Purposes") |
You may be required to supply us with your name, address, phone number, e-mail address, photograph and other personal information requested in the form attached.
If you fail to supply us with such personal data, we may not be able to process and/or disclose your personal data for any of the above Purposes.
Please be informed that Universiti Sains Malaysia may disclose disseminate and/or transfer your information contained in this application to the potential employers/companies and/or other organization/agencies that registered with USM's Resume Bank for and in respect of the Purposes.
Universiti Sains Malaysia is committed in ensuring the confidentiality, protection, security and accuracy of the personal data made available to the University. It is the students' obligation to ensure that all personal data submitted to the University and retained by the University are accurate, not misleading, updated and complete in all aspects.
For the avoidance of doubt, the University and/or its employees shall not be responsible for any personal data submitted by you that is inaccurate, misleading, incomplete and not updated.
You may at any time after the submission of your personal data to the University, submit your request for access to your personal data by submitting such request to the University in writing to the address below in the event:
| (a) | you require access to and/or wish to make corrections to your personal data subject to compliance of such request for access or correction not being refused under the provisions of the Act and/or existing laws; |
| (b) | you wish to enquire about your personal data; and |
| (c) | you are no longer agreeable to us retaining, holding or storing your personal data. However, please note that this may result in us not being able to properly perform or discharge our obligations to you. |
